Every so often I log into my computer and get a notification on the bottom right hand side saying that the device unsuccessfully installed.
I double click it and it says that it is an unknown device.
When I click on it and check for updates, it says windows says the driver is up to date.
Then, my computer crashes and has the BSoD with the words on top with BUGCODE_USB_DRIVER.
I don’t know what is happening, and I did not plug in anything new to my computer from when it started crashing.
It has been happening for about a month now.

My computer is a Toshiba Protégé R705, Intel® Core™ i3 CPU, 2.27GHz

What I have plugged into one of my USB slots is a Dynex 4-port USB hub.
Plugged into that is a LapCool3 Notebook Cooler, a Dell Photo Printer 720, and a Logitech laser mouse.

Even when I unplug all of this, that icon still pops out and says the driver installation is unsuccessful, and when I click it, all the ports still show up even though it is not connected.

To prevent the crash, I usually go to device manager and uninstall/disable that usb device, but windows always brings it back.

I don’t know if this is part of it, but I also notice that when I tilt my screen about 45degree angle (towards closing the screen) the built-in webcam disconnects, and when I put it back upward I hear the connect, disconnect sound about 3 or 4 times until
it stops. Please help!

You can troubleshoot these yourself using BlueScreenView, MyEventViewer and the other methods
in this thread.
Be sure to use MyEventViewer to check at the times of the BSOD's or within a second
before them to see if anything is helpful in the Event Viewer.

Although the Bug Check 0xFE: BUGCODE_USB_DRIVER does usually indicate hardware it could easily
be a USB device attached and not System USB hardware.
And you can never really rule out drivers.

Is this a Upgrade installation of Windows 7 or did Windows 7 come on the system from the factory?
A possible cause could be Windows Updates, especially drivers.
Even drivers not usually thought of as
USB related, such as video/display, network (NIC & WiFI), and others, can be the cause.
Norton especially, McAfee, Kaspersky, SpyBot, Adaware and others could be causing or have a role
in this issue.
Try uninstalling them to see if that helps.
Use their maker's Removal Tools and/or
special instructions.
(Antivirus/antispyware/security programs.)

Be sure to check for updated drivers for ALL USB
devices.

Then lets refresh the USB Stack
Control Panel - Device Manager - click anywhere in the blank/white area - then VIEW - Show
Disabled Devices - look everywhere (except USB Controllers) for any hidden/disabled devices
(may have none or more than one) and Right Click on it - UNINSTALL.

Control Panel - Device Manager - USB Serial Controllers - RIGHT CLICK on EVERYTHING and
UNINSTALL all except the category itself - REBOOT - this refreshes the driver and the USB stack

Here is a utility to help but do the above 1st.

USBDeview is a small utility that lists all USB devices that currently connected to your computer,
as well as all USB devices that you previously used.
Run it Options and check the 1st three
choices to see if any are there.

After the above attach one USB device at a time to see if any cause the issue.

Open Action Center by clicking the Start button Picture of the Start button, clicking Control Panel, and then, under System and Security, clicking Review your computer's status.
